Survivorship (Post-Treatment Care)
Survivorship is an important new phase of life after your treatment has been completed. You'll need advice and guidance as you manage side effects of treatment, regain your energy, and readjust to the routines of everyday life and work. Our survivorship program is dedicated to you and focuses on your long-term care.
Like we did with your treatment, our survivorship program takes a team approach to your post-treatment care. You'll receive an individualized long-term care plan based on your history, diagnosis and treatment. The plan includes:
Survivorship Care Plan/Treatment Summary
Each patient is provided with a summary of his treatment details to keep in his own records and share with other health practitioners as needed.
Follow-up Plan
Many times, patients at other practices are left to navigate post-treatment care on their own. Our concern for you extends beyond your treatment and through your follow-up care. We'll provide you with a comprehensive, detailed plan that identifies future appointments, frequency of visits, and activities and tests associated with each visit, such as lab work.
Erectile and Continence Recovery Program
The Erectile and Continence Recovery Program focuses on providing comprehensive care and a telehealth platform to ensure that men facing prostate cancer not only have the best oncological outcomes, but also the best lifestyle outcomes.
Erectile dysfunction and urinary incontinence are common issues for men following treatment for prostate cancer. The goal of Chesapeake Urology’s Erectile and Continence Recovery Program is the help men regain erectile and bladder function as quickly as possible after prostate cancer treatment by connecting patients to leading specialists in erectile and bladder health management. The recovery coaches at Chesapeake Urology’s Erectile and Continence Recovery Program are dedicated to helping men reach their long-term health goals through education, clinical services, and ongoing support.
Educational Materials
Just as we provided you with detailed information about your cancer diagnosis, treatment options and other valuable resources for your care, we believe it's just as important to supply you with information to help you thrive as a survivor. This includes:
- Long-term effects of treatment and common side effects to watch for, as well as management advice and guidance.
- Links to websites for community support programs that you may find beneficial.
- Our recommendations for healthy living activities to help you maximize your overall health.
Continued Personal Support
You will receive continued support from your patient navigator. Over time, the relevance of your survivor information can change based on your personal situation. Please remember that you can reach out to your navigator as needed. In addition, your entire care team (your urologist, surgeon, radiation oncologist, family physician and our research division CURA) are available. Please contact them as needed to answer any of your healthcare questions and discuss any situation that may come up.
Survivorship Activities
We have a large slate of activities, from our annual Survivorship Picnic and the ZERO Prostate Cancer Run/Walk to opportunities to volunteer and help support other survivors.
